It comes from The Romance of the Rose, a 14th century manuscript. It’s essentially a joke image about how lusty women can be. As it turns out, this isn’t the only time penis trees popped up in medieval art.

Personally, I think I might be more amused by the ambulatory genitalia Pilgrim’s Badge parodies.
